COVID-19 Vaccinations from FCHC:

FCHC will receive weekly shipments of the COVID-19 vaccine.  

The vaccine clinic will be by APPOINTMENT ONLY,  Hours of scheduled clinic will be 1:30 p.m. till 5:30 p.m

Please understand that you will be turned away if you are not registered for an appointment. No swapping of appointments, please register with the name of the person that will be receiving the vaccine. Please bring Identification, all forms accepted (DL, ID Card, Passport, Name Badge etc).

FCHC is beginning to schedule patients that are due for their second vaccine dose on Saturday, February 20th at the Marlin Middle School. This will be a drive through vaccination.

FCHC is receiving a weekly allocation of Pfizer Covid-19 vaccines that will be administered to 1A and 1B patients only.

Due to high call volume, appointments will be scheduled online only through this page.  Please check back. We are in the process of implementing online registration. Once you are able to be registered through this page. You will be scheduled and called with an appointment date and time. 

Please download and complete the VACCINE REGISTRATION FORM prior to your arrival to speed up the vaccination process.

The following groups are currently eligible for this round of vaccinations:

PHASE 1A and 1B:

– Front line medial staff, first responders and last responders

– Persons 65 years of age and older

– Persons 16 years of age and older with at least one chronic medical condition.
