Please be sure to bring your vaccination card with you to the drive thru clinic in order to receive your vaccine. If you do not have your card, you will be scheduled to come to the clinic at a later date
COVID-19 First Dose Vaccinations from FCHC:
FCHC will receive weekly shipments of the COVID-19 vaccine.
The vaccine clinic will be by APPOINTMENT ONLY AND FOR FIRST DOSE ONLY, Hours of scheduled clinic will be 1:30 p.m. till 5:00PM
Please understand that you will be turned away, if you are not registered for an appointment. No swapping of appointments, please register with the name of the person that will be receiving the vaccine. Please bring Identification, all forms accepted (DL, ID Card, Passport, Name Badge etc).
First Dose By appointment only!
Effective Monday March 29,2021 any person 16 years and older is eligible to receive the vaccine with priority given to those persons in the following groups
– Front line medial staff, first responders and last responders
– Persons 65 years of age and older
– Persons 16 years of age and older with at least one chronic medical condition.
– Persons 50 – 64 years of age.
– Teachers and child care workers
